Required Information 所需資料

• The birth year, month, date and time (in Gregorian calendar), gender of the named person.
• 改名者的新曆出生日期、時間、性別
• The Chinese names of parents of the named person. In case of a baby naming, please also provide the Chinese names of grandparents and siblings in order to avoid “same word or same pronunciation”.
• 改名者的父母中文姓名，另如是嬰兒命名，爺爺、嫲嫲、公公、婆婆及兄弟姊妹的中文姓名亦建議一併提供以防「撞字或撞音」
• Specific favorable or unfavorable words or pronunciation (if any).
• 特別喜歡或不喜歡的字或讀音（如有）
• If it is necessary to follow a genealogy, please provide the related information.
• 如需根據族譜改名，請提供所需資料
